Effect of gingival needle design and diameter on pain level of supraperiosteal anesthetic injection

Abstract

Aim: evaluate the level of pain during the application of supraperiosteal anesthetic injections in upper lateral incisors with gingival needles of different diameters, and its correlation with levels of dental anxiety. 

Methods: one hundred and twenty volunteers, aged between 18 and 74 years old, from a Dental School Clinic were randomly distributed into two groups according to the type of anesthetic needle: Septodont XL (n = 60) and Dencojet (n = 60). The level of dental anxiety was measured at the beginning of the consultation using the Corah Dental Anxiety Scale. Participants rated the pain experienced at three phases: during the needle insertion, needle placement, and solution deposition using a Heft-Parker visual analogue scale (VAS HP). Statistical analyzes were performed using the Jamovi 2.3.28.0 software (p < 0,05). 

Results: the pain perceived by female participants (VAS HP = 33.8 mm) was significantly greater (Mann-Whitney U test, p < 0.05) than the pain perceived by male participants (VAS HP = 21.2 mm). No significant differences were found in patients’ dental anxiety and perceived pain between the Deconjet and Septoject XL needles during the needle insertion, needle placement and solution deposition phases (Mann-Whitney U test, p > 0.05). Furthermore, the pain perceived by patients between anesthetic injection phases and age groups was statistically similar (Kruskal-Wallis test, p > 0.05).

Conclusion: that the level of anxiety of patients before the anesthetic injection varied from very little (Dencojet) to mild (Septoject XL), but pain perceived between the Dencojet and Septoject XL needles was similar. Female participants reported higher levels of pain, regardless of the type of anesthetic needle.
